auto or workers compensation insurance carrier.
Billing
services are provided for all major medical plans. OPTM
does not bill secondary insurance companies or third party
carriers. As a courtesy, OPTM will bill supplemental plans
for all Medicare
patients.

provided by OPTM and in no way relieves you of the responsibility.
